broken mysql table?

there is this table that seems to be broken. it is very very strange.. i have even emptied the table completely, .. but whenever i try to insert 1 record, .. 2 records get inserted .

the 2 records are identical however completely different than what i am entering.

it's not just a php script .. this happens directly from the shell

any ideas what can cause a table to do weird things like this?

bad triggers?
faulty replication setup?
filesystem corruption?
mysqld error?
socket hiccups caused by kernel issues?
hacked mysql server?

my guess is someone attempted to own your mysqld, and messed up the current running process. but we're going to need more info.
