Purchase form with Javascript

Hi,

I am new with HTML and Javascript. Could anyone please help me how to create this form as below:

1st Combo Box : Clothes
2nd Combo Box : Dress
3rd Combo Box : Size6
Size8
Size10

The 1st and 2nd combo box would have only one select list. The user will only need to select their choice in 3rd combo box.
The price for Size6 is $25, Size8 is $30 and Size10 is $35.
If user wants to purchase five Size6 dresses, the user will key-in the number 5 inside the empty text box.
Then, another text box will display the total price.
